ELIGIBILITY CRITERIA
  • 1. The annual family income should not exceed Rs. 1.60 lakhs.
  • 2. Government employees and patients with medical insurance are not eligible for this scheme.
  • 3. This scheme is available only for holders of saffron and yellow color ration cards.
  • 4. The scheme applies to patients in general wards only
Note: Applications will be accepted between 4 PM and 5 PM.
DOCUMENTS REQUIRED

FOR CHIEF MINISTER'S RELIEF FUND

  • Application (in the prescribed format)
  • Medical expense certificate for diagnosis and treatment (if from a private hospital, it must be certified by a Civil Surgeon)
  • Income certificate from the Tehsildar office (annual income should not exceed Rs. 1.60 lakhs)
  • Patient’s Aadhar card(Maharashtra State)and mother’s Aadhar card(for children)
  • Patient's Ration card (Saffron or Yellow)— Maharashtra State
  • Relevant disease report
  • For accident cases, a medical-legal case (MLC) report is required.
  • Approval from the Zonal Transplant Coordination Centre (ZTCC) or Government Committee is required for organ transplant cases.
  • MLA/MP recommendation letter (if available)
  • Scan all documents and email the PDF at aao.cmrf-mh@gov.in
  • After emailing, a reference ID will be sent to your registered mobile number.
  • It is suggested to save this ID for future reference.

Important Note:

The benefits of the Chief Minister's Relief Fund are available only during the patient's admission and not after discharge from the hospital. It is advised to complete all procedures while the patient is still admitted.
